Treating vitiligo on the lips and eyelids presents unique challenges. These periorificial sites are highly visible, emotionally significant, and anatomically sensitive. The thin, delicate skin around the eyes and the specialized mucosal tissue of the lips require treatment approaches that differ substantially from those used on the trunk or limbs.

Topical calcineurin inhibitors suppress local immune activity without the skin-thinning effects of corticosteroids — a critical advantage on lip tissue where steroid-induced atrophy would be unacceptable. Calcipotriene (a vitamin D analogue) may further enhance melanocyte activity.

Michael Lewitt, MD, emphasizes that targeted phototherapy can be especially useful in regions like the perioral area where topical therapy is less practical due to moisture and friction from routine activities such as eating and brushing teeth. He recommends nighttime application of topical agents to reduce inadvertent removal, and barrier products like zinc oxide to protect areas where medication should not migrate.

When Medical Therapy Fails

Because lip vitiligo is notoriously resistant, surgical options may eventually be necessary. Suction blister epidermal grafting, miniature punch grafting, and split-thickness grafting have all been used for stable, recalcitrant lip vitiligo. Micropigmentation (tattooing) is another option for patients who cannot achieve satisfactory repigmentation with medical therapy.

Treating Eyelid Vitiligo with Phototherapy

Safety First: What the Research Shows

Eyelid phototherapy has an excellent safety profile when proper precautions are followed. A landmark study on the present status of eyelid phototherapy combined clinical experience with in vitro transmittance data. The researchers found that negligible quantities of UV radiation are transmitted through eyelid skin — in contrast to visible light, where up to 77% of radiation passes through the tissue. This means the eyelid itself provides substantial natural protection to the eye.

In a retrospective analysis of seven patients receiving a combined total of more than 1,300 eyelid phototherapy treatments, no ocular disease from phototherapy was observed. Two case studies demonstrated remarkable improvement without adverse side effects, confirming that incremental UV dosages can be safely delivered to eyelid skin.

When topical therapy alone is insufficient, combining it with targeted phototherapy can significantly enhance results.

Surgical Backup for Resistant Cases

For stable eyelid vitiligo that does not respond to medical treatment, surgical options exist. Suction blister epidermal grafting — where a blister is created on the thigh and the roof is grafted onto the dermabraded eyelid — has shown excellent results. In a study of six patients with stable eyelid vitiligo, response was excellent in all patients within 3 to 6 months, with no complications. The researchers concluded that when performed properly, suction blister technique is the procedure of choice for cosmetically important sites like the eyelids.

The Role of Combination Therapy: Data Support

The evidence for combining phototherapy with topical calcineurin inhibitors is robust across both sites. A systematic review and meta-analysis found that for topical calcineurin inhibitor plus phototherapy, an at least mild response (≥25% repigmentation) was achieved in 89.5% of patients, and a marked response (≥75% repigmentation) was achieved in 47.5% of patients. The use of topical calcineurin inhibitors, both as monotherapy and in combination with phototherapy, should be encouraged in patients with vitiligo.

Importantly, long-term safety concerns have been addressed. A large multicenter retrospective cohort study of 25,694 vitiligo patients found that the long-term risk of skin cancer or lymphoma was not associated with the use of topical calcineurin inhibitors, phototherapy, or both treatments in combination. The interaction between topical calcineurin inhibitors and phototherapy was not associated with an increased risk of skin cancer.

When Phototherapy Is Not Enough

For patients who achieve inadequate repigmentation despite optimal medical therapy, surgical options remain:
